Registering to deliver your baby at Lynden Birth Center
Registering to deliver at LBC is as easy as 1-2-3. All forms, completely signed, must be received by LBC no later than 36 weeks (earlier is better!). And to make it easy on your midwife, please: 1. Print out and fill in the following forms If you do not have access to a printer, please let your midwife or LBC know, and a paper copy will be provided for you. 2. Email them back to LBC if possible (firstname.lastname@example.org) 3. Bring them completed to your midwife to your next appointment for her chart
Lynden Birth Center accepts most insurances, including Apple Health/Medicaid (Molina, CHPW, etc). Many insurances provide coverage for freestanding birth centers in Washington State.
Questions about coverage? 1. Ask your midwife: If you have established care with a midwife, as part of your first appointment, you provided her with your insurance information, which she has sent to her medical biller (often this person is Ingrid at Island Billing, see below). Ingrid runs your benefits and then sends a benefit statement to your midwife, usually takes 3-4 weeks after your initial visit. So, your midwife may have that information. You can ask her at your next appointment.
